It sounds as though you overfaced the horse. As you've found out, it is more important to jump a smaller fence well than a large one badly.
You need to go back to basic to regain your own confidence, and the confidence of your horse. Start small and only increase the size when you are happily cantering over and over the jump without any feelings of trepidation. If that means going right back to trot and canter poles, then so be it.
Many, many show jumpers have been through this (including me!). It may be worth borrowing a schoolmaster or getting lessons on an old plod. You'll be bored before you know it, and back to jumping large and well!
