Have someone else take him out so he gets used to being hacked. If he's stabled a lot he may get over-excited when he hits grass. Lead him out so he gets used to the surroundings and you can really take a good look at the route and get used to it with him.
Keep up with the lessons, try a different instructor though if you don't think you were improving. I started having lessons again a year ago and didn't think I was getting anywhere, but when I look back I am vastly improved - my seat is better, my legs are stiller, I have greater finesse with my aids. You may lack self confidence so have a good chat with an instructor to work out what your problems are and what you want to achieve before you start.
If he pulls on the reins, don't just pull back. Horses are infinitely stronger than we are and if they are given something to pull against then they will. School him in a light contact and work on using your seat and legs to slow him rather than relying on yuor hands. When he's listening to you in the school go out and apply the same tactics. If he pulls, hold with one rein and give little tugs with the other, half-halting basically. If he doesn't respond then tug harder. If you're nervous and he's picking up on it he may not be listening to you which is why he gets faster. You need to get his attention back and that may well have to come in the form of a bloody good yank on his mouth. If he won't slow then get off and carry on leading him. Do not get off and go home or relenet and turn him back otherwise he'll learn that as a really bad habit. Keep going where you were planning to go, then get back on when he's calmer.
Ride out with someone else, preferably on a calm horse so your horse has sensible company. Keep trying, have some confidence building lessons, and focus on the positive instead of the negative. Instead of thinking 'oh, he won't stop, what if....?', think 'well, he was a very good boy cos he stood still when I mounted'. Riding is meant to be fun, make sure you work on achieving this.
