My Welsh Cob is laminitic and should wear a muzzle – but he is determined not to. Sectioning off a small section of field is not an option – is there anything else I could try?

Laminitis is a difficult and often painful condition, and should be managed carefully. Other than check the fit of your muzzle, or look for another design, I feel you will have to restrict the grazing.
Electric fencing is very useful for strip grazing, or you could tether him to restrict the area he can reach. Alternatively, try turning him out in a manège. Restricting grazing would be the best option for the health and welfare of your horse.
