My mum had a 30 odd year old TB x Arab, and she rang up the veteran society for ideas to keep her weight, they gave her loads of ideas, apart from the obvious, layering them up and limiting their exercise so that the fat consumed is not worn off through exercise but built up as body stored fat, also mineral imbalance can cause loss of weight or the animal to be underweight, i'd recommend consulting your vet about what you can do, and whether your horse is getting the vitamins and minerals it needs (no offense intended) , I'll send my mum a link of this post so that she can give you more information about the veteran society, but the advice they gace her worked very well and she did gain a fair amount of weight.
Also, high energy foodstuffs such as sugarbett or quickbeet might help, and limiting either the exercise or the amount of work you are doing with your horse, hope i helped (:
