Before riding go through what your going to do and imagine everything going really well. Set something to work on for each session and focus on it, before getting on sit in a quiet place for a few minutes breathing slowly and calm down, tell yourself it will be good.
When in the saddle, relax and don't tense, your horse will feel it! Tell yourself you are a good rider and you can do it.
Maybe get an instructor, or knowledgable friend to give you a lesson, so you can focus on that and not worry.
As for your horse, if you can maybe get out spooky fillers and let her walk up to them and see they're not scary. Try jumping as part of your every day riding, so she finds it normal and easy when your out. Make sure she knows what you're asking and don't let her get away with being stubborn, however, don't get too cross be understanding and settle her and try again.
If she's to strong maybe think about her feed, is it giving her to much energy? But that's up to you, but remember if you do decide to change it, change it gradually!
Try to get out to lots of places, maybe with an instructor and jump some easy fences and build up from there.
